Your route to the full licence to practise medicine (Approbation) and to a suitable post
Since 1 November 2026, a new procedure has applied to degrees from third countries such as Algeria, Morocco, Tunisia, Syria or Egypt (Drittstaaten: countries outside the EU, the EEA and equivalent states such as Switzerland). Here you will find the procedure, with sources, and medical posts in Germany. If you wish, you can register free of charge.

The route since 1 November 2026
For new applications with a degree from a third country, the knowledge test (Kenntnisprüfung) is the standard route (§ 9d BÄO). The federal states (Länder) are currently putting the new law into practice, and the order of the steps may differ from one federal state (Bundesland) to another.
Applied before 1 November 2026?
For applications submitted by 31 October 2026, the previous law continues to apply to the recognition of third-country degrees (§ 15 BÄO). The licensing regulations (Approbationsordnung) likewise continue to apply in their old version to applications for a full licence (Approbation) or a temporary licence (Erlaubnis) from that period (§ 42 Abs. 2 ÄApprO). Your competent authority can tell you what this means for you.
Degree from the EU, or already recognised there?
This route does not apply to degrees from the EU, the EEA and equivalent states such as Switzerland. It also does not apply if one of these states has already recognised your third-country degree: in that case an aptitude test is provided for where there are substantial differences, and a temporary licence to practise (Berufserlaubnis) is granted only in exceptional cases (§ 9c Abs. 7, § 10 Abs. 1 und 1a BÄO).
As of 6 November 2026. General information, not legal or immigration advice.
Find the competent authority
If you live in Germany, the authority of the federal state (Bundesland) where you have your habitual residence decides on the full licence to practise (Approbation). The federal state where you are to work decides on a temporary licence to practise (Berufserlaubnis) (§ 12 BÄO). These can be two different authorities.
In your application you declare that you are not involved in a procedure for an Approbation or Berufserlaubnis in any other federal state (§ 39c ÄApprO).
Assemble your documents
Among other things, you are required to provide proof of identity, a CV, proof of your training and a certificate showing that you are entitled to practise the profession in the country where you obtained your degree (§ 39c ÄApprO).
The certificate of good conduct (Führungszeugnis), the corresponding documents from the state where you obtained your degree and the medical certificate may be at most three months old when submitted (§ 39e ÄApprO).
For documents not issued in German or English, a translation into German or English by publicly appointed or sworn translators is required. The authority may admit other translators and, in justified exceptional cases, may require a translation into German (§ 33a ÄApprO).
Prove your German
In practice, the federal states (Länder) require general German at level B2 and a medical German test (Fachsprachprüfung) that is based on level C1 (basis: § 3 Abs. 1 Satz 1 Nr. 5 BÄO and the key points of the Conference of Health Ministers of 2014). Each federal state regulates registration and timing itself.
Example: Lower Saxony (Niedersachsen)
€595
is the cost of the medical German test (Fachsprachprüfung) (as of 01.01.2026)
39.07%
of medical German tests (Fachsprachprüfungen) were passed in 2025
Decide within four weeksDeadline: 4 weeks
Instead of going straight to the knowledge test (Kenntnisprüfung), you can choose to have your documents assessed for equivalence (Gleichwertigkeitsprüfung). This is possible only up to four weeks after you submit your application, and the choice binds you for the entire procedure (§ 9d Abs. 2 BÄO).
If the authority finds substantial differences, you must still take a knowledge test (Kenntnisprüfung). This also applies if the application can be assessed only with disproportionate effort because documents cannot be submitted in full for reasons not attributable to you (§ 9d Abs. 3 BÄO). For this choice, further evidence of your training and, where available, of your professional experience is also required (§ 39d ÄApprO).
The authority must inform you about this choice and its consequences. Obtain this information before the deadline expires.
Take the knowledge test (Kenntnisprüfung)
This is an oral and practical examination in German, held on one day, preceded by taking a patient's medical history, a physical examination and a written report. The content covers internal medicine and surgery, as well as emergency medicine, clinical pharmacology, imaging procedures, radiation protection, the legal framework and communication skills. It lasts 60 to 90 minutes per person and can be repeated twice (§ 37 ÄApprO).
The federal states (Länder) must ensure that you can take the test within six months: in the standard route from the date of application, and after an equivalence assessment (Gleichwertigkeitsprüfung) from the formal notice (Bescheid) on the substantial differences (§ 37 Abs. 9 ÄApprO). The authority must issue this notice no later than four months after all documents have been received (§ 38 Abs. 3 ÄApprO). No measured processing times are available for this yet.
Example: Lower Saxony (Niedersachsen)
€980
is the cost of the knowledge test (Kenntnisprüfung) (as of 01.01.2026)
72.4%
of knowledge tests (Kenntnisprüfungen) were passed in 2025
Temporary licence to practise (Berufserlaubnis), if you want to work in a medical capacity beforehand
The authority may grant a licence under § 10 BÄO, but there is no entitlement to one. It is revocable, as a general rule applies only up to a total duration of medical work of no more than two years, and may be restricted to certain activities and places of employment. A decision on the application is to be made no later than three months after the documents have been submitted, and enquiries to other bodies may suspend the deadline (§ 34 ÄApprO).
Some federal states (Länder) impose further requirements. Some of their published information still describes the position before November 2026 and mentions, for example, a passed medical German test (Fachsprachprüfung) (Lower Saxony, Berlin) or proof of an employer for medical work (Hesse). Ask your competent authority about the current position. Your state medical association (Landesärztekammer) can clarify whether periods under a Berufserlaubnis count towards specialist training.
Full licence (Approbation) and specialist training
With the Approbation you may practise medicine without restriction. Specialist training is regulated by your state medical association (Landesärztekammer). It takes place in hospitals and in practices authorised to provide specialist training.
Important in the meantime
Without an Approbation or Berufserlaubnis, you may not practise medicine and may not use the title "Ärztin" or "Arzt" (§§ 2, 2a BÄO, § 132a StGB). Any other practice of the healing arts (Heilkunde) also requires a licence (§ 1 Heilpraktikergesetz).
There is no statutory definition of an observational placement (Hospitation). At its core it means accompanying and observing, without regular work being performed. A time limit and a written agreement are recommended. In the view of the Saxon state medical association (Sächsische Landesärztekammer), individual manual tasks are conceivable only as a delegated service under a doctor's guidance and supervision. Among other things, taking the medical history, examination, diagnosis, informing the patient and the treatment decision remain reserved for doctors. If this turns into regular practical work, the Minimum Wage Act (Mindestlohngesetz) may apply. If in doubt, ask your state medical association (Landesärztekammer).
Costs and recognition grant (Anerkennungszuschuss)
Anyone with their habitual residence in Germany can apply for a grant, subject to income limits (€32,000 gross a year, and €50,000 for married couples and civil partners, in each case less the allowances for children): up to €600 for fees, expenses, translations and certifications in the recognition procedure. Preparatory courses for the knowledge test (Kenntnisprüfung) and examination fees count as financial support for training of up to €3,000. The funding body decides on this only once a notice or message from the recognition body on substantial differences is available. The 2025 guideline gives admission to the knowledge test as an example. Ask the advice centre whether this remains the case in the new standard route. Benefits from the Employment Agency (Agentur für Arbeit) or the Jobcenter take precedence. Apply for the grant before you submit the recognition application, as no costs are reimbursed retrospectively. Applications can be made until 30 June 2027. Language courses, language tests and the costs of the Approbation itself are excluded.

Whether and when you raise this is entirely your decision. Our guides on this are in German:
What SalamJobs does for you and what it does not
What we do
- We show posts where the employer has publicly documented a prayer room or quiet room.
- We keep this procedure up to date and name our sources.
- If we have a post that might match your details, we ask you first. Only with your consent in each individual case do we give the employer or another recruitment agency a short profile. For this, we ask to see your Approbation, your Berufserlaubnis or proof that you have passed the knowledge test (Kenntnisprüfung).
What we do not do
- We do not promise a post, a visa or an Approbation.
- We do not promise placement. Registering gives rise to no entitlement to contact, a job suggestion or an introduction.
- We do not assess individual cases and do not submit applications. Authorities and advice centres are there for that.
- We do not ask about religion, headscarf, gender, nationality or the country of your degree.
- We do not target people abroad and do not place people from abroad. Only people who already live in Germany can register.
How we earn money
If, with your consent, we introduce you to an employer or another recruitment agency and you start work with that employer, the employer or the agency pays us a fee. For you it remains free of charge. For the employer, however, it can make you more expensive than a direct application. That is why you decide in each individual case whether you would like to apply yourself or be introduced.
Register
We get in touch by email if a post might match your details or if we have a question. We can introduce only people who can prove that they hold an Approbation or a Berufserlaubnis or have passed the knowledge test (Kenntnisprüfung). We are at an early stage, so it may take time, or it may happen that we do not get in touch.
What people often ask
What does it cost?
Nothing for you, neither registering nor an introduction nor being hired. Payment comes only from the employer side or a recruitment agency.
Can I also apply without registering?
Yes. Every post links directly to the employer. You do not need us for that.
I do not live in Germany yet. Can you help me?
At present you cannot register. If you do not have your habitual residence in Germany, you submit the application to the authority of the federal state (Bundesland) where you wish to work (§ 12 BÄO). The Service Center for Professional Recognition (Zentrale Servicestelle Berufsanerkennung) offers guidance.
May an agency ask me for money?
A placement agent may charge job seekers a fee only if an employment contract has come about through its placement. It may not exceed €2,000 including VAT, an advance payment is not permitted, and the placement contract must be in writing (§ 296 SGB III, exceptions apply, for example in the case of a placement voucher). SalamJobs charges you nothing.
Where can I get binding information?
From the authority responsible for your Approbation and from your state medical association (Landesärztekammer). You can find free recognition advice through the Recognition Portal (Anerkennung in Deutschland), which also hosts the Recognition Finder (Anerkennungs-Finder).
